All Athletic Registrations must be completed by the PARENT/GUARDIAN and the STUDENT together.

Winter 2024-2025 sports registration is Oct. 4-Nov. 1, 2024

High School Winter Sport tryouts begin 11/25/24

Middle School Basketball tryouts are the week of Nov. 11th.

Your child must be registered and medically cleared to try out.

Medical Eligibility Forms and health history forms should be turned into your building's nurses' office. 
Only hard copies of the physical and health history forms are accepted. No electronic delivery will be accepted. 
DO NOT email the Medical Eligibility or health history form.
Uploading it to your registration portal does not deliver to us. That is only seen by you. 

If you miss the deadline to turn in your paperwork, we cannot guarantee clearance for the first day of practice. This is a firm deadline.

***Please keep the start date in mind when submitting your paperwork. Physical must be current through the start date.*** 

Example: HS Athletes must have a physical exam date of 11/25/23 or later. Otherwise, a new physical/Medical Eligibility form on file is required. 

MS must have a physical exam date of 11/25/23 or later. 

If physical is current, only the health history update (dated within 90 days of the start of the season) is required but we must have the current physical evaluation form/Medical Eligibility form on file.

PLEASE READ!!! VERY IMPORTANT!!!!

ONE ACCOUNT PER FAMILY!!

Registration is a two-step process and takes about 15 minutes so please follow these instructions:

 
Step 1 - Register your student online.  If a student isn’t certain in which sport she or he would like to participate, parents should still register for the sport in which some interest has been expressed. The selection can always be changed prior to the beginning of the new sport’s tryouts with written parental consent.
PLEASE NOTE: If this is the first time you are registering a student, you will be asked to set up a Family Account. By doing so, future registration forms will be pre-filled for your convenience but will allow you to make changes and complete any new fields that have been added. Once a registration form has been submitted, a confirmation page will be displayed indicating that Step 1 has been completed. You will NOT receive a confirmation email that your child is registered until the physical has been reviewed and your child has been medically cleared. 
If you have another student to register for a sport, please log into your account and then select the Athletic Registration Form so all your children are associated with your one Family Account. Do not create a second family account for your additional children. It will be deleted and you'll have to do it all over again on the original Family Account. 
 
Step 2 - Pre-Participation Physical Evaluation/Medical Eligibility Form and Pre-Participation Physical Evaluation Health History Form
Exams after July 1, 2024 must be on the NEW Medical Eligibility Form!!!
 
Every Student needs a Health History Update dated within 90 days of the start of that season prior to EACH SEASON
If you don't know your child's last physical date, call the school nurse or your family doctor. 
If it's expiring before the start of the season, then a new physical/Medical Eligibility Form must be turned in to the nurse. If we already have the physical on file and is up to date, then only the health history form is required.

Before a student/athlete may try-out, practice, scrimmage and/or participate in a game, the items below must be satisfied – NO Exceptions!
        1.  All students must read the Parent/Student Athlete Handbook (available inside the online activity registration form)
2.  All students must meet academic requirements.
3.  To begin participation on a sport team, a student must be cleared by the Athletic & Activities Director.
 
Registration dates for 2024-2025
Spring 2025 sports registration is January 29 - February 28, 2025
All HS spring sports begin March 10, 2025
MS sports TBD (around 3/17)
